It is a typical Portuguese communal flag, with the coat of arms centred on a plain yellow field.

Coat of arms

Shield Azure with base wavy of Argent and Vert, charged with a fish Gules; an impending fortress Argent with an embattled wall and two turrets, masoned sable with port and windows Gules; in chief a corn cob Or, leafed Argent. Mural crown Argent with three visible towers (village rank) and white scroll with inscription in black capitals "PORTO FORMOSO".
Meaning:
The commune's name means "fair haven" or "beautiful port". The base wavy is referring to the location on the shores of the Atlantic. The fish is symbolising fishery, the corn cob agriculture. The fortress is representing Forte de Nossa Senhora da Graça (fortress of Our Lady of Grace), sheltering the Bay of Porto Formoso.

Porto Formoso Commune is one of the 14 communes of Ribeira Grande Municipality; it had 1265 inhabitants in 2011 and covers 11.5 km².
