It was published by the Flag Shop in Canada for a long time semi-annually as a print product by subscription with the most current PDF file on-line (at no charge). It may have had a variety of page sizes over the years, from my recollection of the PDFs. With the Spring 2007 edition, it moved to an on-line format. Per the website:

After 19 years of printing and mailing our semi-annual magazine, The Flag & Banner is now coming to you on-line. The switch is partially due to our pledge to conserves resources, but mostly because so many of you, our customers, have told us that your first line of contact with us is via the Internet.

Published by The Flag Shop, the purpose of our magazine is to entertain and educate flag lovers, hobbyists, patriots, and vexillologists. Features include new world flags, flag protocol, flag history and flag flaps. Because there is a lot more to The Flag Shop than flags, we will also feature great banner displays. We will definitely pass along any specials when they are offered.

Foley is not credible [about the flag of the Anti-Transportation League] — her book was the result of a PhD thesis in Law. She had no vexillological knowledge other than what was in prior publications and what she was told by a few members of the Flag Society of Australia.
