It is a typical Portuguese communal flag, with the coat of arms centred on a plain red field.

Shield Or, on a mound Vert a pig Argent emphasized Sable and eyed Gules, atop the mound an embattled Roman fortress (lit.: a castrum-like building) Gules masoned Argent. Mural crown Argent with three visible towers and white scroll reading in black upper case letters "VILARINHO DO MONTE".
Meaning:
"Vilarinho" means something like "small settlement" and "monte" means "hill", which makes the arms partly canting. However the whole toponym is a miniaturisation for a nearby village and commune seat, Vilar do Monte, to which it was named (one of many such cases in Portugal). I have no specific info about any additional symbolism for the pig and the fortress, apart from the obvious.

Vilarinho do Monte Commune was one of the pre-2013 communes of Macedo de Cavaleiros Municipality; it had 67 inhabitants in 2011 and covered 7.3 km².
