Founded in 1877 by Count Tsunetami Sano, it took its preset name in 1887 and was recognized as such by the International Committee of the Red Cross that same year. (Source)

The Hakuai Sha (Japan Philanthropy Corporation) was established in 1877 by Tsunetami
Sano. Because Japan didn't adhere to International Red Cross
in 1877 the flag was used for 10 years until 1887, when Japan joined the International
Red Cross.

The flag used by the Hakuai Sha was a combination of an army hospital flag and the national flag; not only was Japan was not part of the Geneva Convention but also some saw the main Red Cross sign as religious in nature. Also, during the time of conflict, it was hard to introduce new (and possibly confusing) symbols to those seeking aid. It was decided to against using the Red Cross in favor for this flag, which was using an already established emblem. (Source)
