The municipality of Šmarješke Toplice was inaugurated on 1 January 2007, following a referendum held on 29 January 2006 in the local
communities of Bela Cerkev and Šmarjeta, then part of the municipality of Novo mesto.

The flag and arms of Šmarješke Toplice are prescribed by decision Odlok o grobu i zastavi Občine Šmarješke Toplice, adopted on 26 June 2007 and published on 6 July 2007 in the official Slovene gazette Uradni list Republike Slovenije, No. 60.
The flag is a banner of the municipal arms, that is, the same design in rectangular form. The vertical flag is also prescribed, with the design properly rotated.

The coat of arms of Šmarješke Toplice is "Per fess vert and azure overall a cross argent ensigned with a jar or".

Quoting the municipal website:
The jar represents hospitality, as water-filled jars would be left atop tables to quench the thirst of travelers. A different version of this jar had an important place in each cottage. Ours is round and cannot stand alone, so it has two handles, that it might be held with both hands, and purposefully directed as we direct our lives, not allowing others to steer them for us. Let us be masters of our lives. The circle represents the sun or the ripe grape berry.
The field, divided by a vertical white band, represents the wine-growing areas with their respective waters: Vinji Vrh with the [river] Krka and Kogla with the Radulja. Some may also interpret them as the two former local communities. The divided blue box represents thermal and potable waters. The four implied fields can symbolize fire brigades, and the circle the municipal firefighting association.
