It is a typical Portuguese communal flag, with the coat of arms centred on a field quartered of yellow and green.

Coat of Arms

Shield Or a barry wavy of five of Vert and Argent between grape bunches Purpure leafed Vert in fess, three in chief and three in base. Mural crown Argent with four visible towers (village rank) and white scroll reading in black upper case serifed letters "VILA DE SANFINS DO DOURO".
Meaning:
The arms are partly canting with the wavy fess symbolising the River Douro.
The communal territory does not confine with the Douro, though, which lies further to the south (forming the southern border of the encompassing Alijó municipality), rather having its tributary River Pinhão flowing along its western border (which is also the municipal border here). The toponym includes the name of the better known river (and its basin/province) as a disambiguation against the several other settlements similarly named — and I presume that this is what the wavy fess refers to.
The wavy fess being Vert instead of Azure might be a case of armas a inquirir, an invitation to further investigation, a kind of Heraldic Easter egg — as green waves in Portuguese Heraldry usually mean the sea, with rivers being symbolised as blue waves, and the sea is 136 km away.

Presentation of Sanfins do Douro

Sanfins do Douro was one of the ten communes of Alijó Municipality not affected by the 2013 changes ; it had 1256 inhabitants in 2021 and covers 17.09 km².
