It is a typical Portuguese communal flag, (see left image above) with the coat of arms centred on a field quartered of white and blue.

Other depictions of this flag (see right image above) unsurprisingly follow this interpretation with two shades of blue, such as the above mentioned embroidered patch by Lousã Têxtil.
The flag on the photo also seems to be a blend of what is prescribed separately for the banner (estandarte) and for the hoisting flag (bandeira [de hastear]), as it is oblong at 2:3 but corded,
with tassles and fringes, and sleeved to a spear.

Shield Azure, a craggy mound Argent, parted by a bar wavy Azure, crowned by a castle of the same masoned Sable with port and windows Gules having three embattled towers.
The mountain is flanked by a branch of olive fructed Sable at dexter and a branch of oak fructed Vert at sinister both Or,in chief a bar wavy Argent.
Mural crown Argent with four visible towers (town rank) and white scroll with inscription in black initials "MONTALVÃO".

Meaning:
The word "montalvão" derives from the phrase "monte alvo_" (being, by surface analysis, juxtaposed and in the augmentative), meaning "white
hill" — these arms being therefore canting.

Montalvão Commune is one of the seven communes of the Nisa Municipality; it had 442 inhabitants in 2011 and covers 123.9 km².
