FINA was founded in 1908, and now has 171 countries affiliated with it. The flag is 1:2; a light blue-white-light blue tribar. In the upper light blue tribar a white world map in two parts near hoist. The flag was introduced in 1908.
The protocol manual for the London 2012 Olympics (Flags and Anthems Manual
London 2012 [loc12]) provides recommendations
for flag designs. Each international federation
and international paralympics federation was sent an image of the flag,
including the PMS shades, for their approval by LOCOG. Once this was obtained, LOCOG
produced a 60 x 90 cm version of the flag for further approval. So, while these
specs may not be the official version of each flag, they are certainly what the international
federation and international paralympics federation believed the flag to be.
For FINA: PMS 3015 blue, 1235 yellow. The vertical flag is simply the horizontal
version turned 90 degrees clockwise.

The flag shown here was also depicted and detailed in the "Flag Manual" by the
Beijing Organizing Committee for the Games of the XXIX Olympiad, 2008. It is
depicted 2 x 3 (horizontal) and 3 x 2 (vertical).
Colors (PMS): Process Blue
Remarks: FINA changed emblem in 2008. Probably there exists a new flag.

During the opening ceremony the flag of the competions was also flown joinly
with the flag of FINA and that of Italy.
The flag of the competition was white charged with the official logo.

On 16 July 2011 were inaugurated the FINA World Aquatics Championships in
Shanghai, China PR, at the Shanghai Oriental Sports Center.
The flags parade was in alphabetical order after the country's codes. The
only exceptions were Italy which marched first, and
China which marched
last as host.
Ghana and Kuwait marched under the FINA flag
Netherlands Antilles (AHO) still exits, at least to FINA. It marched under its
flag in second placed behind Italy
and before Albania.
Oceania and Europe only missed one of its members each, Samoa (SAM) and
Gibraltar (GIB) respectively.
I counted 181 contries and "regions" as called by FINA, perhaps it is a new
record. In Rome 2009, GIB, TCN, IVB, took part. But I don't know the exact
number of countries.

According to this
page at Roma 2009, 200 nations were involved. But in reality, as mentioned
here, there
were 179 participating countries. Excelsior.
