Photos of the flag and a description are available on this Waymarking page.
https://www.waymarking.com/waymarks/WMY768_Columbia_Illinois_Municipal_Flag:
"The flag is of a white field. In the center is a red map of the State of Illinois with a small five pointed star on the center left edge of the map denoting the location of Columbia, Illinois. Superimposed on the map is a black and white drawing of a Bavarian man in lederhosen steadying an orange Alpine Horn. The date - 1859 with small lines above and below appear to the left of the state map; to the right, the date - 2009 with the same small lines above and below. A black circle encompasses all of the above. At the top and around the Circle are the words: "Columbia, Illinois" and below and around the circle are the words: "150th Anniversary · Celebration 2009" in orange."

An image of the large German fellow, which I traced to draw the flag, was obtained from the city's Facebook page: https://www.facebook.com/knowcolumbia/photos/a.433777038076/10153914887248077
