It is a typical Portuguese communal flag, with the coat of arms centred on a field quartered of white and red.

Coat of Arms

Shield Gules with base wavy of Argent and Azure, on fess point a mitre Argent garnished Azure with scarves of the same, superimposing a mitre and a cross staff in saltire, both Or, on dexter chief a railway profile Or, on sinister chief a grape bunch Or leafed Argent. Mural crown Argent with four visible towers (town rank) and white scroll reading in black upper case serifed letters "FREGUESIA DE PENAFIEL".
Meaning:
The enlarged commune of Penafiel is located between the rivers Sousa and Cavalum, both being right tributaries of the Douro River and represented by the base wavy with two blue barrulets wavy. The mitre, which also appeared on the pre-2013 arms, is alluding to the fact that Penafiel still is seat of a titular bishopric. The railway profile is taken from the arms of the former Novelas Commune, where the local railway station is located. The grape bunch, a symbol of viticulture, had been part of all former coats of arms, except Santiago de Subarrifana and Novelas.

Shield Gules with base wavy of Argent and Azure; issuant from base a throughout bridge Or, masoned Sable and having three arches; above two railway profiles Argent ordered per fess, in chief an orb Or. Mural crown Argent with three visible towers (village rank) and white scroll with inscription in black capitals "NOVELAS - PENAFIEL".
Meaning:
The orb is an attribute of Jesus Christ, as the Divine Saviour being the local patron saint.

Shield Argent with base wavy of Azure and Argent, a dolmen Gules, in chief a cogwheel Azure, flanked by two grapevines Or leaved Vert. Mural crown Argent with three visible towers (village rank) and white scroll with inscription in black capitals "SANTA MARTA - PENAFIEL".
Meaning:
The so called "Dolmen of Sta. Martha" is a national monument since 1916. The grape bunches are symbolising agriculture, the cogwheel industries. The base wavy is a representation of the Cavalum River.
